A person with medial tibial stress syndrome has strained the tibialis posterior muscle. The tibialis posterior muscle is a large muscle, located in the front of the lower leg. It raises the ankle and turns the ankle inward.

Overuse causes the muscle to become irritated and inflamed. Medial tibial stress syndrome is most common in those who run regularly.
