The evaluation of epididymitis begins with a medical history and physical examination.

Physical findings in males with epididymitis include:

• Penile discharge
• Testicular swelling
• Testicular tenderness (overlying the epididymis)
• Inguinal lymphadenopathy:
• Swollen and tender lymph glands in the groin
• Penile sore
• Rash

The diagnosis of epididymitis is made during physical exam. In some cases, tests may be necessary to exclude conditions that can cause symptoms similar to epididymitis.

Tests that may be used to evaluate epididymitis include:

• Gonorrhea testing
• Chlamydia testing
• Herpes genitalis testing
• HIV testing
• HPV testing
• Syphilis testing
• Urinalysis
• Urine culture
• Doppler ultrasound scan of the scrotum
• Nuclear testicular scan
