The evaluation of diabetic neuropathy starts with a history and physical examination.

Physical findings in someone with diabetic nephropathy may include:

• Skin breakdown or ulcers:
• Usually on the feet
• Weak pulse in the ankles and feet
• Skin changes due to peripheral vascular disease
• Decreased sensation in the feet and legs
• Leg numbness (bilateral)
• Feet numbness

Testing may be necessary to assess the severity of the diabetic neuropathy.

Tests that may be used to evaluate diabetic neuropathy include:

• Complete blood count
• Glycosylated hemoglobin (hemoglobin A1c)
• Vitamin B12 level
• Folic acid level
• Drug screen
• VDRL:
• Blood test that screens for syphilis
• Electromyogram
• Nerve biopsy
• Nerve conduction studies

Less common tests that may be used to evaluate diabetic neuropathy include:

• Lumbar puncture with spinal fluid analysis
• CT scanning of the brain
• MRI scan of the brain
